- certifies that the hours shown are correct and that the work was performed in a satisfactory manner; and
- confirms the Client’s agreement to the terms and conditions stated on the reverse side of this form.
- Four (4) hours minimum per day required.
- Overtime (time and a half is paid after forty (40) total hours worked
